Medical Appointment Requests

You can submit a Medical Issue Request online via Accurx between 7:30 am and 4:30 pm on weekdays.

Use the online medical request form to request an appointment. A Triage GP will assess your request and contact you accordingly. Please provide as much detail as possible to help us process your request efficiently.

Your request will be carefully reviewed. We may schedule an appointment for you or direct you to a more appropriate service.

You may also be offered an evening or weekend appointment with our enhanced hours team.

 

Admin Requests

To submit an administrative request, select the Admin Query section and complete your submission.

Our Practice Team will review your request and get in touch with the outcome.

Please note: Medical requests should not be submitted through the admin query option.

 

Your appointment

We will provide the most appropriate type of appointment based on your request. This may be:

  • By phone

  • Face-to-face at the surgery

  • On a video call

  • Via text or email

While we understand that patients may have a preferred consultation method, this may not always be suitable.

To help us book the right appointment, please provide as much detail as possible when submitting your request. This allows us to assess your needs and ensure you receive the best care.

GP Extended Hours

We are now open on Tuesday and Friday evenings, as well as every fourth Saturday of the month, as part of the Government’s initiative to enhance healthcare access.

This service is designed for routine medical concerns and is not an emergency service. Some conditions may not be suitable for an extended hours appointment. Our trained reception team can guide you on what can be booked within this service.

Available appointments include GP consultations, nurse reviews, smear tests, and blood tests. These appointments take place at The Park Medical Centre and other local GP surgeries.

This service is open to all registered patients.

If you require urgent care after 6:00 pm, please call NHS 111 for advice or visit A&E in an emergency.

How does the online booking system work at The Park Medical Centre?

Our online consultation system is available from 7:30 am to 4:30 pm on weekdays.

What if my appointment is urgent but there are no available slots?

If you feel you need to be seen today, you can:

  • Call reception to check for cancellations.

  • Call NHS 111 for urgent medical advice.

For genuine emergencies, please go to A&E or call 999 immediately.

What if my medical query is non-urgent?

Please submit a new request using the Medical Issue option from 7:30 am on the next working day. A member of our team will respond within 48 hours.

What if I do not have internet access or struggle to use online services?

If you cannot access the internet:

  • Call reception, and our team will submit a request on your behalf.

  • Visit the surgery, and a staff member will assist you.

For vulnerable or housebound patients, we can arrange a home visit if there is sufficient medical need.

How can I provide feedback on the appointment system?

We value your feedback and continuously strive to improve our service. You can:

  • Leave feedback on NHS Choices.

How do I cancel my appointment?

If your appointment is within the next 12 hours, use the cancellation link in the reminder text you received. If you haven't received a text, call reception at 0161 998 5538.

For all other appointments:

  • Use the cancellation link in the appointment reminder sent 1-3 days before your appointment.

  • If you didn’t receive a text reminder, cancel via our online request system (using the Admin Request option).

  • If you cannot cancel online, call reception for assistance.

I am housebound and need to arrange a home visit

To request a home visit, please call the practice as early as possible on the day. Your GP will visit if there is sufficient medical evidence that you cannot attend the surgery.
